Privacy Policy The Social Welfare Department (SWD) of the Government of the Hong Kong Special Administrative Region (the Government) is concerned to ensure that all personal data submitted through the Long Term Care Services Delivery System website (LDS website) are handled in accordance with the relevant provision of the Personal Data (Privacy) Ordinance. The LDS website is a restricted website exclusively provided for use of authorised personnel of recognised referring offices, subsidised community care service / residential care service providers and accredited assessors under Standardised Care Need Assessment Mechanism of Elderly Services in handling registration and allocation of subsidised long term care services for the elderly applicants in Hong Kong. Your personal data supplied by your serving agency/office including your full name, user ID, email address and post will be securely kept in the servers of the Long Term Care Services Delivery System for authentication of you identity when you login to the website. Your full name will be printed on your output LDS Form for authorship and the system will issue notification to your email account to inform you of incoming LDS Form. Apart from the above, the Government will not disclose your personal data to any third parties without your prior consent unless permitted or required by law. You have the right to request access to your personal data held by SWD in accordance with the Personal Data (Privacy) Ordinance 486. A fee is charged for supplying copies of personal data. As your personal data held in the Long Term Care Services Delivery System was supplied by your serving agency/office, please approach your agency/office for correction of your personal data.
